 THE FOLLOWING ITEMS ARE MISSING FROM THE APPLICATION
 SUBMITTAL AND THEREFORE THE APPLICATION IS DEFICIENT.
  
 1) COMMUNICATIONS FACILITIES PLACEMENT PERMIT
 APPLICATION CITY RIGHTS-OF-WAY - THE APPLICATION LISTS
 COLLOCATION ON EXISTING TOWER AS THE TYPE OF FACILITY;
 HOWEVER, THE DETAILS OF THE ANTENNA INSTALLATIONS WERE
 NOT PROVIDED. PLEASE PROVIDE A BRIEF NARRATIVE OF WHAT
 IS PROPOSED SO THAT STAFF CAN VERIFY THE TYPE OF
 FACILITY THAT YOU WILL BE INSTALLING.
  
 2) ENGINEERING PLANS, SIGNED AND SEALED BY FLORIDA
 REGISTERED PROFESSIONAL ENGINEER OR EXEMPT, 10FT, 20FT,
 OR 40FT SCALE:
  
 A. THE ENGINEERING PLANS WERE SUBMITTED AT A 1 INCH -
 50 FEET SCALE, WHICH DOES NOT COMPLY WITH THE CITY
 SCALE REQUIREMENT LISTED IN THE APPLICATION. PLEASE
 PROVIDE THE PLANS AT THE REQUIRED SCALE.
  
 B. THE ENGINEERING PLANS MUST SHOW THE PROPOSED
 RIGHT-OF-WAY IMPROVEMENTS FOR THE CLARE AVENUE
 RESIDENTIAL PROJECT, THE PROPOSED AND UNDERCONSTUCTION
 ROSEMARY AVENUE RECONSTRUCTION PROJECT AND THE
 UNDERCONSTRUCTION QUADRILLE/FERN MAST ARM INSTALLATION
 PROJECT. STAFF NEEDS TO VERIFY THAT THE PROPOSED FIBER
 CONDUIT, MARKER POLES, HANDHOLDS AND FUTURE
 COMMUNICATION POLES DO NOT INTERFERE WITH THESE
 APPROVED, PROPOSED AND CONSTRUCTED RIGHT-OF-WAY
 PROJECTS.
  
 C. PROVIDE COPIES OF SHEETS 19 THROUGH 22, WITH THE
 SURVEY BACK UP LISTED IN COMMENT 3, SO THAT STAFF CAN
 VERIFY THAT THE PROPOSED WORK IS IN THE RIGHT-OF-WAY.
 THE LOCATION MAP LINE APPEARS TO CROSS OUT OF THE
 RIGHT-OF-WAY LINE ONTO PRIVATE PROPERTY.
  
 D. THE PROPOSED PROJECT INCLUDES A NUMBER OF HANDHOLDS.
 THE DETAILS ON SHEET 09 ARE UNCLEAR AS TO WHAT THE
 HANDHOLD WILL LOOK LIKE AT GRADE. WILL THE LID OF THE
 HANDHOLD BOX BE VISIBLE AT THE GRADE, OR WILL IT BE
 COVERED WITH ANOTHER MATERIAL ? SOD, CONCRETE, PAVERS?
 PROVIDE A NARRATIVE TO ADDRESS. PLEASE NOTE THAT THE
 RIGHT-OF-WAY IMPROVEMENT PROJECTS LISTED IN 2B. MAY
 CHANGE THE SURFACE MATERIAL WHERE THE PROPOSED HANDHOLD
 IS LOCATED.
  
 3) SURVEY OF UNDERGROUND FACILITIES - NO SURVEY WAS
 PROVIDED.
  
 A. PROVIDE A SURVEY SHOWING THE PROPERTY LINES AND ANY
 TREES WITHIN THE RIGHT-OF-WAY ALONG THE FIBER CONDUIT
 PATH.
  
 B. THE ENGINEERING PLANS MUST SHOW THE SAME CONDITIONS
 AS THE SURVEY, SO THAT STAFF CAN VERIFY THAT NONE OF
 THE PROPOSED WORK (MARKER POLES, HANDHOLDS OR CONDUIT
 IS BEING INSTALLED ON PRIVATE PROPERTY AND THAT NO
 TREES (SPECIFICALLY THEIR ROOT SYSTEM) ARE BEING
 AFFECTED.
  
 4) INVENTORY OF REGISTRANT'S EXISTING FACILITIES - NO
 INFORMATION ON EXISTING FACILITIES WAS PROVIDED. SHOW
 THIS INFORMATION ON THE PLANS AND PROVIDE A NARRATIVE.
  
 5) CONCEALMENT METHODS DESCRIPTIONS - NO INFORMATION ON
 WHERE THE MARKER POLES WILL BE LOCATED AND TO HOW THEY
 WILL BE CONCEALED WAS PROVIDED. SHOW THIS INFORMATION
 ON THE PLANS AND PROVIDE A NARRATIVE.
  
 6) ESTIMATED SCHEDULE - NO SCHEDULE WAS PROVIDED.
 PROVIDE A NARRATIVE OF THE ESTIMATED SCHEDULE OF THE
 PROPOSED WORK.
  
 7) POLE PLACEMENT INFO AND MAKE-READY ESTIMATE - NO
 INFORMATION WAS PROVIDED ON THE LOCATION OF THE FUTURE
 POLES. SHOW THIS INFORMATION ON THE PLANS AND PROVIDE A
 NARRATIVE.
  
 8) PROOF OF MAILING NOTICE TO OWNERS, ONE COPY OF
 ADDRESS LIST OF PROPERTY OWNER'S ADDRESSES WITHIN 250
 FEET AND ONE COPY OF RADIUS MAP PROVIDED BY THE PAPA
 SHOWING THOSE PROPERTIES WITHIN 250 FEET OF AFFECTED
 RIGHTS-OF-WAY - THE APPLICATION CHECK LIST INDICATES
 THAT THIS WAS DONE (AS WELL AS THE AFFIDAVITS YOU
 PROVIDED), YET NONE OF THE REQUIRED DOCUMENTS WERE
 SUBMITTED TO THE CITY. PLEASE CLARIFY AND PROVIDE THE
 INFORMATION.
  
 9) ELECTRONIC FORMAT OF SUBMITTAL - ONLY THE PLANS WERE
 PROVIDED ON THE DISC. A COPY OF ALL OF THE SUBMITTAL
