 1) THIS PROJECT WAS PREVIOUSLY SUBMITTED AS WCF ROW
 PERMIT 20080824. STAFF RECEIVED AN EMAIL FROM THE
 APPLICANT ON NOVEMBER 19, 2020, REQUESTING THAT THE
 APPLICATION BE VOIDED, WHICH RESULTED IN THE PLANS,
 INCLUDING THE SURVEY, BEING DISPOSED OF. PLEASE SUBMIT
 THE SURVEY, PREPARED BY A FLORIDA LICENSED SURVEYOR,
 FOR THE LENGTH OF THE PROJECT WHICH SHOWS RIGHT-OF-WAY
 LINE, SIDEWALKS, EDGE OF PAVEMENT AND TREES, SO THAT
 STAFF CAN VERIFY THAT THE WIRELESS COMMUNICATION
 FACILITY, INCLUDING THE UNDERGROUND CONDUIT, HANDHOLES
 AND BORE PITS, IS WITHIN THE RIGHT-OF-WAY AND NOT ON
 PRIVATE PROPERTY AND AVOIDS TREE ROOT ZONES.
  
 2) COMMENTS FROM THE ORIGINAL SUBMITTAL NOTED THAT THE
 ORIGINAL ENGINEERING PLAN (STAFF NO LONGER HAS A COPY)
 DID NOT MATCH THE SURVEY IN SHOWING THE LOCATION OF THE
 RIGHT-OF-WAY LINE ON SHEETS 19, 22, 23, 25, 26, 27, 29,
 30 AND 31 AND THERE WERE HANDHOLES THAT SHOWED POSSIBLY
 OVERLAPPING ONTO PRIVATE PROPERTY. THE CORRECT THE
 RIGHT-OF-WAY LINE LOCATION MUST BE SHOWN ON THE
 RESUBMITTED ENGINEERING PLANS.
  
 3) THE FLAGLER DRIVE RIGHT-OF-WAY ADJACENT TO GOOD
 SAMARITAN HOSPITAL DOES NOT APPEAR TO BE SHOWN
 CORRECTLY ON THE ENGINEERING PLANS BASED ON A PRIOR
 CITY APPLICATION WHICH INCLUDED A SURVEY. YOUR SURVEYOR
 WILL NEED TO SHOW THE CORRECT EASTERN RIGHT-OF-WAY LINE
 LOCATION. PLEASE NOTE THAT THE RIGHT-OF-WAY LINES THAT
 THE PALM BEACH COUNTY PROPERTY APPRAISER SHOWS ON THEIR
 AERIALS ARE NOT BASED ON SURVEYS, SO THEY SHOULD NOT BE
 USED TO INDICATE THE RIGHT-OF-WAY LINES. THIS AFFECTS
 SHEETS 26, 27 AND 28.
  
 4) THE CITY IS IN THE PROCESS OF REDESIGNING CURRIE
 PARK. THE SURVEY PROVIDED WITH THAT PROJECT SHOWS THE
 RIGHT-OF-WAY LINE IN A DIFFERENT LOCATION (FURTHER
 WEST) THAN THE ENGINEERING PLAN YOU HAVE SUBMITTED.
 PLEASE NOTE THAT THE RIGHT-OF-WAY LINES THAT THE PALM
 BEACH COUNTY PROPERTY APPRAISER SHOWS ON THEIR AERIALS
 ARE NOT BASED ON SURVEYS, SO THEY SHOULD NOT BE USED TO
 INDICATE THE RIGHT-OF-WAY LINES. CURRIE PARK STARTS ON
 YOUR SHEET 32 AND ENDS AT YOUR SHEET 37. UNTIL YOU
 PROVIDE YOUR ENGINEERING PLAN BASED ON THE SURVEY
 REQUIRED IN COMMENT #1, STAFF CANNOT VERIFY IF THE
 PROPOSED BORE, BORE PITS OR HANDHOLES ARE WITHIN THE
 RIGHT-OF-WAY. NOTE: FROM COMPARING PLANS IT APPEARS
 THAT SHEETS 34, 35 AND 36 SHOW ENCROACHMENT ONTO
 PRIVATE PROPERTY, WHICH IS NOT PERMITTED.
  
 5) THE PLANS SHOW AN ELECTRIC LINE WITHIN 5 FEET OF THE
 BORE AND THE BORE PITS IN MANY LOCATIONS, WITH A
 HANDHOLE AT LESS THAN 2 FEET FROM THE ELECTRIC LINE. IN
 ORDER TO VERIFY THAT THE INSTALLATION WILL NOT
 INTERFERE WITH THE EXISTING FACILITIES, PROVIDE A
 PUBLIC UTILITY LETTER OF NO OBJECTION FROM FPL.
  
 6) THE PROFILE VIEWS STATE THAT THEY ARE SHOWN LOOKING
 EAST, WHEN THEY ARE ACTUALLY DRAWN LOOKING WEST. PLEASE
 CORRECT THE LABEL.
  
 7) PROVIDE A COPY OF THE FDOT PERMIT ALLOWING THE BORE
 ACROSS QUADRILLE AVENUE.
  
 8) BORE PITS 61+90 AND 67+10 ARE LOCATED TOO CLOSE TO
 EXISTING PALM TREES. THEY MUST BE AT LEAST 10 FEET FROM
 THE TRUNK OF THE TREE. THIS DIMENSION NEEDS TO BE
 PROVIDED ON THE PLAN.
  
 9) THE PROPOSED BORE, INCLUDING SOME HANDHOLES AND BORE
 PITS, ARE LOCATED CLOSE TO THE EXISTING BULKHEAD. THE
 BULKHEAD UTILIZES A VARIETY OF STRUCTURAL
 REINFORCEMENTS, SUCH AS TIE BACKS AND CONCRETE DEADMAN.
 THE CITY DOES NOT HAVE EXACT LOCATIONS OF THE
 REINFORCEMENT FACILITIES, SO YOU WILL NEED TO SOFT DIG
 EVERYTHING THAT WILL BE INSTALLED BETWEEN GRADE AND
 FIVE (5) FEET BELOW GRADE AND WITHIN 20 FEET OF THE
 SEAWALL. THE PROFILE VIEWS SHOW THE BORE AT "MINIMUM 48
 INCHES" WHICH IS NOT A SUFFICIENT DEPTH TO AVOID THE
 SEAWALL STRUCTURAL REINFORCEMENTS. THE BORE NEEDS TO BE
 SHOWN DEEPER TO AVOID THESE FEATURES.
  
 10) THE BORE SHOWN AT 4 FEET BELOW GRADE IS AT A
 SUFFICIENT DEPTH TO AVOID TREE ROOT DAMAGE.
  
 11) ABOVE GROUND TUBULAR MAKER POLES ARE NOT PERMITTED.
 IF THE HANDHOLES NEED TO BE MARKED, THEN THE MARKER
 MUST BE LOCATED WITHIN THE HANDHOLE.
