 EASEMENT. UNLESS THE SURVEY SHOWS A GENERAL UTILITY
 EASEMENT AT THIS LOCATION, YOU MAY NOT CROSS THE

 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.

 2) ENGINEERING PLAN -
  
 A. SHEETS 11 AND 11A INCLUDE NOTES THAT STATE "HH TO HH
 1516 FEET BY TRENCH" AND ALSO "HH TO HH 1516 FEET BY
 DBORE (DIRECT BORE)". PLEASE EXPLAIN WHY THE PLANS LIST
 TWO DIFFERENT METHODS FOR THE SAME SECTION, YET THE
 DRAWINGS SEEM TO ONLY SHOW THE BORE.
  
 B. PLEASE EXPLAIN WHY THE CROSS SECTION DRAWINGS SHOW
 THE BORE AT 6-7 FEET IN DEPTH, YET THE TEXT REFERENCES
 THAT THE DEPTH WILL BE MINIMUM OF 36 INCHES.
  
 C. THE BORE PATH ON SHEET 13, JUST BEFORE REACHING
 MATCH LINE STATION 14+91 IS SHOWN VERY CLOSE TO THE
 ADJACENT PROPERTY LINE AND THUS NO LONGER IN THE
 RIGHT-OF-WAY. EITHER THE SURVEY NEEDS TO SHOW THAT
 THERE IS A UTILITY EASEMENT ON THE PRIVATE PROPERTY, OR
 YOU NEED TO CLEARLY SHOW THAT THE BORE DOES NOT EXIT
 THE RIGHT-OF-WAY.
  
 D. THE BORE PATH ON SHEET 23, BY THE SOUTH MATCH LINE
 THAT HAS THE BORE VERY CLOSE TO PRIVATE PROPERTY.
 EITHER THE SURVEY NEEDS TO SHOW THAT THERE IS A UTILITY
 EASEMENT ON THE PRIVATE PROPERTY, OR YOU NEED TO
 CLEARLY SHOW THAT THE BORE DOES NOT EXIT THE
 RIGHT-OF-WAY.
  
 E. THE PLAN SET INCLUDES A SHEET SHOWING ABOVE GROUND
 MARKER POLES, WHICH ARE NOT PERMITTED. IF THE HANDHOLES
 NEED TO BE MARKED, THEN PROVIDE THE INFORMATION ON THE
 HANDHOLE COVER OR WITHIN THE HANDHOLE AS HAS BEEN SHOW
 IN OTHER APPROVED PLAN SETS.
  
 3) ELECTRONIC FORMAT OF SUBMITTAL - A COPY OF ALL OF
 THE SUBMITTAL MATERIAL MUST BE SUBMITTED IN ELECTRONIC
 FORMAT.
