 PROVIDE WRITTEN RESPONSE TO COMMENTS.
  
 THE FOLLOWING ITEMS NEED TO BE ADDRESSED OR THE PERMIT
 WILL NOT BE APPROVED.
  
 1) ENGINEERING PLAN -
  
 A. THE ENGINEERING PLAN DOES NOT MATCH THE SURVEY IN
 SHOWING THE LOCATION OF THE RIGHT-OF-WAY LINE ON SHEETS
 19, 22, 23, 25, 26, 27, 29, 30 AND 31. WHILE THE FIBER
 INSTALL IS CLEARLY IN THE RIGHT-OF-WAY, THERE ON SHEET
 31 IT LOOKS LIKE IT MAY CROSS THE RIGHT-OF-WAY LINE;
 HOWEVER, ADJUSTING THE RIGHT-OF-WAY LINE TO MATCH THE
 SURVEY APPEARS TO CORRECT THIS ENCROACHMENT. IT IS NOT
 CLEAR THAT ALL OF THE HANDHOLE PLACEMENTS ARE WITHIN
 THE RIGHT-OF-WAY GIVEN THE RIGHT-OF-WAY LINE
 DISCREPANCY. ADDITIONALLY, SHEET 31 DEFINITELY SHOWS
 THAT TIE IN 84 +75 INCLUDES A PORTION ON PRIVATE
 PROPERTY. CORRECT THE RIGHT-OF-WAY LINE LOCATION ON THE
 RESUBMITTED ENGINEERING PLANS.
  
 B. THE BORE SHOWN AT 12 FEET BELOW GRADE IS AT A
 SUFFICIENT DEPTH TO AVOID TREE DAMAGE.
  
 PLEASE NOTE:
  
 THE PROPOSED BORE, INCLUDING SOME HANDHOLES AND TIE
 INS, ARE LOCATED CLOSE TO THE EXISTING BULKHEAD. THE
 BULKHEAD UTILIZES A VARIETY OF STRUCTURAL
 REINFORCEMENTS, SUCH AS TIE BACKS AND CONCRETE DEADMAN.
 THE CITY DOES NOT HAVE EXACT LOCATIONS OF THE
 REINFORCEMENT FACILITIES, SO YOU WILL NEED TO SOFT DIG
 EVERYTHING THAT WILL BE INSTALLED BETWEEN GRADE AND
 FIVE (5) FEET BELOW GRADE AND WITHIN 20 FEET OF THE
 SEAWALL.
  
 THE INSTALLATION CONTRACTOR MUST CONTACT SUNSHINE 811
 TO VERIFY THE LOCATION OF ALL UTILITIES AND VERIFY ALL
 DEPTHS PRIOR TO CONSTRUCTION. ALL CITY UTILITIES MUST
 BE LOCATED BY SOFT DIG METHODS BEFORE STARTING THE
 WORK.
  
 THE RIGHT-OF-WAY USE PERMIT REQUIRES YOU TO PROVIDE A
 CERTIFICATE OF LIABILITY INSURANCE, WHICH IN ACCORDANCE
 WITH SECTION 78-406 OF THE CITY CODE SPECIFICALLY NAMES
 THE CITY OF WEST PALM BEACH, ITS COMMISSIONERS,
 OFFICERS, EMPLOYEES AND AGENTS AS ADDITIONAL INSURED.
