 FACILITIES PLACEMENT PERMIT APPLICATION, YOU MARKED
 THAT THIS IS A PASS THROUGH PROVIDER, YET YOU HAVE ALSO
 MARKED THAT YOU ARE A COMMUNICATION SERVICES PROVIDER
 THAT REMITS TAXES TO THE CITY. BY CITY DEFINITION, YOU
 CANNOT BE BOTH. IF YOU ARE A PASS THROUGH PROVIDER, YOU
 ARE REQUIRED TO PAY AN APPLICATION FEE TO THE CITY.
  
 13) POLE OWNER APPROVAL - THE APPROVAL FROM THE OWNER
 OF THE POLE PERMITTING THE INSTALLATION OF A FACILITY
 WAS NOT PROVIDED. PLEASE NOTE THAT IF THIS IS BEING
 INSTALLED ON A CITY POLE, THEN FEE IS REQUIRED TO BE
